All accidents, incidents and hazards are to be reported PROMPTLY to either:

The Hazard and Incident Report Form is available for download from the OHS Forms website. You may begin to complete the report yourself.
The Safety Officer completes the Incident Report Form, which is then forwarded onto the University’s OHSE Unit.
The Safety Officer investigates instances and make recommendations to prevent recurrences so prompt reporting may prevent further occurrences.
Work Cover forms are available from the Health Service, Campus Centre.
